$330.00 per wrestler $300.00/wrestler if 10 or more from same school or club attend. Coaches who assist attend free of charge. Please sign up early: Registrations received after August 1, 2008 add $25 for late sign-up. Health/Medical Insurance is the responsibility of the camper's parent or guardian. This form is set up as a convenient way for you to send us each camper's information. To complete the registration process, please mail a $100 deposit check or full payment after you submit this form. Make check payable to: DOAC - Grizzly and Mail to: 187 Aspetuck Ridge Road, New Milford, CT 06776 Deposits are made and accepted as a mutually binding token and cannot be refunded after July 1. In the event of withdrawal prior to July 1 a processing fee of $50 will be deducted from the amount refunded. The balance of the tuition is due by July 1, 2008. No reduction in fee can be made for non-arrival, late arrival or early departure. The director reserves the right to expel a camper, without refund, who in her/his discretion, has behaved in a manner contrary to the best interests of the camp. In the case of campers who, as a result of accident or illness incurred at camp, are absent from camp, arrangements will be made with the parent or guardian for prorated refund. I approve of my Camper’s attendance at wrestling camp and certify that he/she is in good health and able to participate in all camp activities. If medical attention is required for illness or injury while attending camp, I give my permission for such care and I hereby waive and release the Camp, its staff, and Delaware Outdoor Adventure Center of all liabilities for any illnesses or injuries that may occur. I hereby give the Grizzly Wrestling Camp/Delaware Outdoor Adventure Center permission to use any photograph, video or likeness of my child. I have read the enrollment terms stated above and agree to their conditions.
